I want to add pdf markers with strikethough text. Is there a way to do so with cpdf?

Bookmarks as added by -add-bookmarks are unicode text strings. It's up to the PDF viewer to decide how to show them, e.g what font to use, and which characters are available.

You could use something like

As a side note testing annotation has very similar limitations so here both pieces of blue text are identical its applied but displays without as it's not a normal appearance and also in this case gets truncated !
