Put the select in the first form, 2 it is ok to form combined
In the form submitted by default way will refresh the page after click the query button, can't get form2 select values,

Why must use the form? It is difficult to use, write directly input and button, click on the submit, write their own ajax requests,

That need not the default submit submit, ajax to submit, change the type=submit to type=button, so that all values to want oneself to get splicing json format or formData format
